Application for a vehicle licence to transport dangerous goods

This form is used to apply for a vehicle licence to transport class 2 to 9 in accordance with Regulation 171 to 175 of the Transport of Dangerous Goods by Road and Rail (National Uniform Legislation) Regulations 2011.
