Section 74 of the Work Health and Safety (National Uniform Legislation) Act 2011, requires a person conducting a business or undertaking to prepare, display and maintain a list of Health and Safety Representatives (HSRs) and Deputy HSRs in their workplace and provide that list to the Regulator (NT WorkSafe) as soon as practicable after its preparation.
The HSR notification form can be used to assist in meeting this obligation; however, it is not mandatory to use the form and the information can be provided in a different written format such as in an email: firstname.lastname@example.org or by letter: GPO Box 1722, Darwin NT 0801.
